Testosterone enanthate is a long-acting ester derivative of testosterone, the primary endogenous androgen in humans. It is synthesized by esterifying the 17β-hydroxyl group of testosterone with heptanoic acid, a modification that enhances lipophilicity and extends its biological half-life. Similar to testosterone cypionate, it is practically insoluble in water and is commonly formulated as an oily injectable solution. After intramuscular injection, the lipophilic ester is gradually hydrolyzed by tissue esterases to release free testosterone, which binds to androgen receptors to exert its androgenic and anabolic effects. Its sustained release characteristic simulates the physiological secretion rhythm of endogenous testosterone, maintaining stable serum hormone levels for a long time.
Testosterone enanthate is mainly used in androgen replacement therapy (ART) for adult males with primary or secondary hypogonadism, aiming to restore and maintain normal serum testosterone levels. It is also indicated for the treatment of delayed puberty in adolescent males with hypogonadotropic hypogonadism or constitutional delay. In addition, it can be used off-label to improve muscle wasting caused by chronic diseases such as AIDS and cancer, but such use must be under strict medical monitoring. Administered intramuscularly every 2–3 weeks (adjusted according to individual patient response), it effectively maintains muscle mass, bone density, sexual function and overall quality of life in patients with testosterone deficiency.
